Your First Visit

Please allow for a 1-2 hour consultation appointment.  This consultation will allow for a thorough & detailed history of your current condition(s).  Please come prepared with a current and complete medication list, any drug allergies, and any other pertinent medical information.  Dr. Thakur will formulate a treatment plan specific to your needs.  Testing may or may not be required, and that will also be discussed and determined based on your needs.

The new patient packet can be completed prior to your appointment, this can be found under the “Forms” link.

Insurance

We advise that you check with your insurance provider that Dr. Thakur is in network.

We also ask that you bring your insurance card with you to your appointment, as well as notify us immediately if there have been any changes to your insurance.

Allergy Injection Hours:

Contact our office for the current office hours for different locations.

Allergy Injections are walk-in, first come first served.  Please remember, we kindly request you stay for 30 minutes following your injection as a safety precaution.
